1.X-rays and PET scans by providers – Ambulatory

Description: This report gives detail of all the X-ray and PET scan orders placed by providers in a given time frame and shows if a patient had the procedure performed. If a patient has not had a procedure or schedule for one the radiology staff would follow up with patients.

Users: Radiology department staff

2.Patient lists for all the PCP’s with HEP C diagnosis – Ambulatory

Description: Generated lists of patients with HEP C diagnosis for around 200 PCP’s. The lists evaluate if a patient (baby boomers) has ever been diagnosed with HEP C or had a screening for HEP C. Baby boomers who have not been screened for HEP C and had a previous diagnosis of HEP C are sent follow up letters to get a screening for HEP C.

3.Provider’s Inbasket messages – Ambulatory

Description: This report gives the count of provider’s Inbasket messages which are read, new, completed and in pending status. Report has start date, end date of message received and provider id. Report can be run for multiple providers at the same time. Report is used to track the Inbasket message response or completion time of providers.

4.Procedure transactions charges posted – Billing

Description: This report shows the procedure charges posted within 10 days after service date and after 10 days and within 30 days of service date. The main report shows the procedures posted within 10 days and 1 sub report shows the procedures posted after 10 days and within 30 days. The 2nd sub report shows the count of transactions posted within 10 days and after. Report has start date, end date for service parameters. This report is used to assess the department staff’s lag in posting the charges.

5.Meaningful use provider score cards – Ambulatory

Description: Generated score cards for providers for meaningful use measures. Also, generated detail reports using SQL queries for each measure to analyze why providers are failing certain measure.

Users: Quality team

6.Distress Management Flowsheet data – Ambulatory

Description: This report displays the information from the Distress Management flowsheet template recorded in a patient’s chart. This report helps to understand how a patient is feeling about his life and performing his daily activities.This report has start date, end date and location parameters.
